DocuSign Envelope ID: 97E04238- A5D4- 41EE- ACB6- DDF028BB5823 <br />Proposal For: <br />Orange County Health Department <br />Attn: Pam McCall and Rebecca Crawford <br />300 West Tryon St. <br />Hillsborough, NC 27278 <br />Dear Pam and Rebecca: <br />Starpoint Global Services ( "Starpoint ") hereby submits this proposal for the digitization of <br />miscellaneous paper -based medical records belonging to Orange County Health Department <br />( "OCHD ", "the Health Department "). These records are split between locked file cabinets on the <br />first floor of the Hillsborough clinic and in boxes stored in three cages as the county storage site. <br />Thank you for giving Starpoint the opportunity to provide you with this information. <br />Executive Summary <br />The Orange County Health Department operates two clinics in Orange County, one in Chapel <br />Hill and one in Hillsborough. Starpoint has already scanned all active records from both clinics <br />and loaded this data to Patagonia. <br />The remaining paper files include a variety of document types such as TB files, communicable <br />disease, staff medical, rabies, flu and older pediatric records which remain within legal retention <br />requirements. <br />Starpoint proposes to scan these records, indexing by names, dates of birth and record type. A <br />most recent date of service could also be added. Digital files will be delivered utilizing a logical <br />naming convention constructed from the index values, such as Doe_ John A_mm- dd- yyyy_2016 <br />Communicable. We can also sort the digital files into separate folders by record type. The files <br />will be text searchable, as well. <br />Starpoint will pack all remaining hardcopy files from the cabinets for relocation to Starpoint's <br />Siler City facility. There will be no cost for empty containers or for labor to pack them. We will <br />then work with clinic staff to identify each of the boxes from the storage cages that will be <br />designated for scanning, leaving behind all records that are already eligible for destruction, as <br />well as any that can be destroyed within the next few years. <br />Starpoint will image all records using Digitech Systems, Inc. (DSI) PaperFlow software. Images <br />will be captured using Bowe Bell +Howell Spectrum & Plus Scanners at 200 dpi, utilizing the <br />
