c) Are not presently indicted for or otherwise criminally or civilly charged by a governmental entity <br />(Federal, State, or local) with commission of any of the offenses enumerated in paragraph 2. (a) <br />(b) of this certification. <br />d) Have not within a three -year period preceding this application had one or more public transaction <br />(Federal, State, or local) terminated for cause or default. <br />e) Agree to include a term or condition in lower tier covered transactions requiring lower tier <br />participants to comply with subpart C of the OMB guidance in 2 CFR part 180, as supplemented by <br />subpart C of Part 417. <br />Where the applicant is unable to certify to any of the statements in this certification, he or she shall attach <br />an explanation to this certification. <br />3. DRUG -FREE WORKPLACE (GRANTEES OTHER THAN INDIVIDUALS) <br />As required by the Drug -Free Workplace Act of 1988, and implemented at 2 CFR Part 182, Subparts B, <br />and C, for grantees: <br />The applicant certifies that it will: <br />a) Make a good faith effort, on a continuing basis, to maintain a drug -free workplace. You must <br />agree to do so as a condition for receiving any award covered by this part. <br />b) Publish a drug -free workplace statement and establish a drug -free awareness program for your <br />employees (see Sections 182.205 through 182.220); and <br />c) Take actions concerning employees who are convicted of violating drug statutes in the workplace <br />(see Section 182.225), including notification to any Federal agency on whose award the convicted <br />employee was working and within 30 days take appropriate personnel action against the <br />employee, up to and including termination, consistent with the requirements of the Rehabilitation <br />Act of 1973 (29 U.S.C. 794), as amended; or require the employee to participate satisfactorily in a <br />drug abuse assistance or rehabilitation program approved for these purposes by a Federal, State <br />or local health, law enforcement, or other appropriate agency. <br />d) You must identify all known workplaces under your Federal awards (see Section 182.230). <br />The grantee must provide the location site(s) for the performance of work done in connection with the <br />specific grant. <br />Place(s) of Performance (Street address, city, county, state, zip code) <br />501 W.
