99 I <br />indicate the amount of Foundation funding <br />requested and how such funds will be used over <br />the period requested. The budget should also <br />include actual and anticipated funds from other <br />sources that also will be devoted to accomplishing <br />the project's objectives. <br />g. IRS Letter or Evidence of Tax - Exempt Status — <br />Organizations that have received from the IRS a letter <br />of determination of exemption should attach a copy <br />of the determination letter. Organizations awaiting <br />a determination of exemption 1hould attach written <br />evidence that a determination has been applied for. <br />A governmental unit should attach a copy of statutory <br />or other authority evidencing that it is a unit of <br />government. <br />h. Major Contributors — Provide a list of the <br />organization's major contributors and the amounts <br />contributed (if applicable). <br />i. Volunteer Services — Provide a brief description of <br />volunteer involvement and a list of in -kind <br />contributions (if applicable). <br />Agreement to Use Funds for Exempt Purposes — <br />Attach a statement of agreement to use the funds <br />exclusively for charitable, scientific, educational, or <br />other exempt public purposes, signed by an <br />authorized officer of the organization or a public <br />official authorized to sign on behalf of the <br />governmental unit. <br />4. Signatures — The application must have an original <br />signature of two officers who have been legally authorized <br />to commit the organization, including the signature of <br />the chief executive officer of the organization. <br />S. Application Submittal —The deadline for responding to <br />the current announcement application submittal is October <br />16, 2000, 4:00 p.m. <br />Please note that incomplete or late applications will <br />be ineligible if in the judgment of the Board missing <br />information is essential to the review of the appli- <br />cation and /or the late submission unfairly advantages <br />the applicant over other applications sent on time. <br />Electronic or faxed copies will not be accepted. <br />Applications should be mailed to the following address: <br />The Golden LEAF Foundation <br />4825 Creekstone Drive <br />Suite 200 <br />Durham, NC 27703 <br />to financial audit requirements. Applicants must submit <br />documentation of capability to meet those requirements. <br />Documentation might include, for example, audited <br />financial statements, IRS letter, etc. <br />14'.4"♦ E° A� ve u h appl avi.T oll bi, s <br />The Foundation will engage a panel(s) of independent <br />reviewers to evaluate applications for activities /projects <br />submitted by eligible organizations for funding under this <br />Announcement. Final decisions on all awards are the <br />responsibility of the Board. <br />For applications to be considered for funding under this <br />Announcement, applicants must meet the eligibility criteria <br />(see What Organizations are Eligible ?), and applications must <br />be postmarked (or other proof of transmission by the <br />deadline) by the application deadline. <br />Applications will be evaluated in terms of the following criteria: <br />I . The potential for the outcomes of the proposed project/ <br />activity to contribute significantly to the long -term <br />sustainable economic and social development of <br />economically affected or tobacco- dependent regions in <br />North Carolina. <br />2. The extent to which the proposed project /activity is <br />directly related to one or more of the seven (7) activities <br />listed previously (see What Activities May the Foundation <br />Fund ?) <br />The extent to which outcomes and lessons learned are <br />transferable to regions outside the proposed project/ <br />activity region. <br />4. The extent to which community participation is involved <br />in project /activity planning and implementation. <br />5. The extent to which Foundation funds will be used to <br />attract or supplement funds from other sources and to <br />foster collaborations with other organizations, thereby <br />leveraging these funds and increasing their impact. <br />6. The extent to which the applications contain all the <br />required information and follow the proposal instructions <br />(see Proposal Instruction Package). <br />7. The capacity of the organization to complete the <br />proposed projectlactivity on time and within budget. <br />8. Previous related experience of the organization and the <br />proposed staff. <br />7. Cost Accounting Requirements — Successful 9.
