The budget should also include actual and anticipated funds from other sources that also will be devoted to accomplishing the project's objectives. g. IRS Letter or Evidence of Tax - Exempt Status — Organizations that have received from the IRS a letter of determination of exemption should attach a copy of the determination letter. Organizations awaiting a determination of exemption 1hould attach written evidence that a determination has been applied for. A governmental unit should attach a copy of statutory or other authority evidencing that it is a unit of government. h. Major Contributors — Provide a list of the organization's major contributors and the amounts contributed (if applicable). i. Volunteer Services — Provide a brief description of volunteer involvement and a list of in -kind contributions (if applicable). Agreement to Use Funds for Exempt Purposes — Attach a statement of agreement to use the funds exclusively for charitable, scientific, educational, or other exempt public purposes, signed by an authorized officer of the organization or a public official authorized to sign on behalf of the governmental unit. 4. Signatures — The application must have an original signature of two officers who have been legally authorized to commit the organization, including the signature of the chief executive officer of the organization. Final decisions on all awards are the responsibility of the Board. For applications to be considered for funding under this Announcement, applicants must meet the eligibility criteria (see What Organizations are Eligible ?), and applications must be postmarked (or other proof of transmission by the deadline) by the application deadline. Applications will be evaluated in terms of the following criteria: I . The potential for the outcomes of the proposed project/ activity to contribute significantly to the long -term sustainable economic and social development of economically affected or tobacco- dependent regions in North Carolina. 2. The extent to which the proposed project /activity is directly related to one or more of the seven (7) activities listed previously (see What Activities May the Foundation Fund ?) The extent to which outcomes and lessons learned are transferable to regions outside the proposed project/ activity region. 4. The extent to which community participation is involved in project /activity planning and implementation. 5. The extent to which Foundation funds will be used to attract or supplement funds from other sources and to foster collaborations with other organizations, thereby leveraging these funds and increasing their impact. 6. The extent to which the applications contain all the required information and follow the proposal instructions (see Proposal Instruction Package). 7.
