The Board will <br />consider the recommendations and approve the final action steps for the 2016-2018 <br />Strategic Plan. <br /> <br />The draft plan is included as an attachment to this abstract. <br /> <br />Background <br />Based on the Community Health Assessment completed in 2015, the Orange County <br />Board of Health identified three priority areas to work on over the next four years. They <br />are: <br />1) Social Determinants of Health <br />2) Physical Activity & Nutrition <br />3) Substance Abuse & Mental Health <br />Three subcommittees of Board of Health members and OCHD staff determined the <br />action steps based on progress in the 2014-2016 plan and the community’s priorities, <br />and will work over the next two years to complete the activities on the new plan. <br /> <br />Recommended Action: _x__Approve <br /> ___Approve & forward to Board of Commissioners for action <br /> ___Approve & forward to _______________ <br /> ___Accept as information <br /> ___Revise & schedule for future action <br /> ___Other (detail):
