3/14/2016 <br />2 <br />BOH Requirements for Accreditation <br />Benchmark 38: The local board of health shall participate <br />in the establishment of public health goals and objectives. <br />Activity 38.1 –the local board of health shall annually by <br />the LHD on the community’s health (e.g. SOTCH & CHA) <br />Required documentation:Minutes reflecting board of <br />health’s review of annual reports related to the <br />community’s health <br />BOH Requirements for Accreditation <br />Activity 38.2 –The local board of health shall review <br />community health assessment data and citizen input used <br />to plan and monitor progress toward health-related goals. <br />Documentation –Board of health minutes reflecting <br />discussion of specific aspects of CHA data AND the approval <br />of action plans to address health related goals. <br />BOH Requirements for Accreditation <br />Activity 38.3 –The local board of health shall assure that <br />individuals, agencies, and organizations have the <br />opportunity to participate in the development of goals, <br />objectives and strategies for community health <br />improvement. <br />Documentation –Board of health policy regarding public <br />participation in the development of goals, objectives and <br />strategies for community health improvement AND board <br />of health minutes reflecting that public participation <br />occurred or agenda indicating allocated time to encourage <br />public participation. <br />Benchmark Activities <br />To satisfy Benchmark 38: <br />Activity 38.1 <br />Review of annual reports (i.e. 2015 CHA) <br />Activity 38.2 (review CHA data and citizen input): <br />Citizen input was received through surveys, focus groups, listening sessions <br />and during prioritization process and board participation was requested by <br />Liska Lackey during the March 2015 BOH Meeting. <br />Participated on the CHA committee’s (CHALT and Community Engagement) <br />Participated in Community Listening Sessions <br />Participated in HCOC’s Annual Meeting and prioritization process <br />Activity 38.3 <br />Assure participation in goal, objective, and strategy development (i.e. <br />community health action plans) <br />Dissemination Requirements <br />Benchmark 1 <br />Activity 1.3: The local health department shall disseminate <br />results of the most recent (2015) CHA to the local health <br />department’s stakeholders, community partners and the <br />general population. <br />Documentation: Evidence of CHA dissemination efforts by at <br />least 2 methods directed to stakeholders/community <br />partners AND the general population <br />Examples: Website, newsletter, news releases, meeting minutes <br />describing the presentation, etc. <br />Next Steps <br />1.CHA approval and dissemination <br />2.Continue and finalize community health action plans <br />around the 3 priority areas and present to you for <br />approval. <br />1.Will work through June with intentions to begin <br />implementation July 2016.
