<br /> <br />Orange County Community Health Assessment <br />Focus Group Discussion Guide <br /> <br />INTRODUCTION <br /> <br /> Thank you for taking the time to join us today. <br /> <br /> INTRODUCE YOURSELF, NOTETAKER(S) <br /> <br />THE FOLLOWING SCRIPT IS FOR YOU TO SUMMARIZE. YOU DO NOT NEED TO READ IT WORD <br />FOR WORD. YOU DO NEED TO COVER CONFIDENTIALITY AND THE RIGHT TO WITHDRAW <br />WITHOUT PENALTY. <br /> <br />I am working with the Orange County Health Department and Healthy Carolinians of Orange <br />County, and we are interested in learning about the health of Orange County residents. Today <br />we would like to hear what you think about the overall health of your community (i.e. physical, <br />mental, and environmental). The information that you share, information gathered from <br />community surveys, other discussions and existing statistics, will help us plan future programs <br />that better meet the needs of residents of Orange County. <br /> <br />No names will be attached to any of the information we collect. We will share what we learn <br />with community and agency members during open forums in the fall. In the winter we will <br />write a report about our county’s health, to submit to the state. If you would like to be invited <br />to a community forum, please write your name and contact information on the sign-up sheet. <br />(INCLUDED WITH DEMOGRAPHICS SHEET) <br /> <br />While we talk today, I want you to feel free to share your opinions even if they are different <br />from others and to react to each other's thoughts. There is no right or wrong answer. I am here <br />to help facilitate the discussion and listen to what you have to say. (NOTETAKER’S NAME) <br />_________ will be taking notes. If there are no objections, we will be recording this discussion <br />to make sure we do not miss any comments. Try and speak up so the recorder can pick up your <br />answer. After this discussion, we will listen to the recording and write down all of the <br />responses, and then we will erase/destroy the recording. Since this is a group discussion, you <br />do not have to wait for me to call on you to speak. Anything we say here is confidential. I ask <br />that when you all leave today that you remember to respect others' privacy and not share any <br />information outside of this discussion. We will talk for about 1 hour. <br /> <br />You are here because you voluntarily agree to participate in this group discussion. However, if <br />for any reason you feel uncomfortable and do not want to continue in the discussion, you are <br />free to withdraw at any time. This will not affect, in any way, the services you receive in the <br />future from Orange County. Again, no names will be attached to the information that we <br />collect. Is this OK with everyone?
