<br /> <br />Orange County Community Health Assessment <br />Focus Group Discussion Guide <br /> <br />INTRODUCTION <br /> <br /> Thank you for taking the time to join us today. <br /> <br /> INTRODUCE YOURSELF, NOTETAKER(S) <br /> <br />THE FOLLOWING SCRIPT IS FOR YOU TO SUMMARIZE. YOU DO NOT NEED TO READ IT WORD <br />FOR WORD. YOU DO NEED TO COVER CONFIDENTIALITY AND THE RIGHT TO WITHDRAW <br />WITHOUT PENALTY. <br />I am working with the Orange County Health Department and Healthy Carolinians of Orange County, <br />and we are interested in learning about the health of Orange County residents. Today we would like to <br />hear what you think about the overall health of your community (i.e. physical, mental, and <br />environmental). The information that you share, information gathered from community surveys, other <br />discussions and existing statistics, will help us plan future programs that better meet the needs of <br />residents of Orange County. <br /> <br />No names will be attached to any of the information we collect. We will share what we learn with <br />community and agency members during open forums in the fall. In the winter we will write a report <br />about our county’s health, to submit to the state. If you would like to be invited to a community forum, <br />please write your name and contact information on the sign-up sheet. (INCLUDED WITH <br />DEMOGRAPHICS SHEET) <br /> <br />While we talk today, I want you to feel free to share your opinions even if they are different from others <br />and to react to each other's thoughts. There is no right or wrong answer. I am here to help facilitate the <br />discussion and listen to what you have to say. (NOTETAKER’S NAME) _________ will be taking notes. If <br />there are no objections, we will be recording this discussion to make sure we do not miss any <br />comments. Try and speak up so the recorder can pick up your answer. After this discussion, we will <br />listen to the recording and write down all of the responses, and then we will erase/destroy the <br />recording. Since this is a group discussion, you do not have to wait for me to call on you to speak. <br />Anything we say here is confidential. I ask that when you all leave today that you remember to respect <br />others' privacy and not share any information outside of this discussion. We will talk for about 1 hour. <br /> <br />You are here because you voluntarily agree to participate in this group discussion. However, if for any <br />reason you feel uncomfortable and do not want to continue in the discussion, you are free to withdraw <br />at any time. This will not affect, in any way, the services you receive in the future from Orange County. <br />Again, no names will be attached to the information that we collect.
