8 <br />a Educate families regarding the importance of accessing routine health services through a <br />"medical home" primary health care provider. <br />Q Assist providers and parents with the development of individualized emergency medical plans <br />for children with identified health issues (such as seizures, asthma, bee sting allergies, etc.), <br />ensuring a system of communication exists among child care providers, parents, and medical <br />providers when health issues arise. <br />The person staffing this project will be a Public Health Nurse II working within the Orange County <br />Health Department and assigned to the Personal Health Services Division/Family Home Visiting <br />Services Section. Beckie Hermann, RN, Family Home Visiting Services Coordinator, will be the <br />supervisor for this position as a part of the team of 3 Public Health Nurse II and 4 Social Worker II <br />home visitors she currently supervises. Currently .25 FTE is funded through OCPYC to provide <br />emergency care plan development for young children with special health needs as a part of the Child <br />Service Coordination Enhancement Project at Orange County Health Department. As the Child Care <br />Health Consultant orients to the project, the currently funded activity will continue this aspect of the <br />position through June 30, 2005. It is planned that this specific duty will transition to the Child Care <br />Health Consultant's role as of July 1, 2005. One of many positive aspects of this position being part of <br />the Orange County Health Department is that there is a Medical Director on staff who oversees and <br />assists with writing of all health related protocols. Another positive aspect is that the Health <br />Department has years of experience providing School Nursing services, immunization review, <br />identification and linking of children with special needs to early intervention services, and providing <br />Communicable Disease investigation and intervention.
