link families with special need children to early intervention or other needed services <br />assess access to preventive health care for children and help link families to services in <br />the community <br />collaborate with and provide education as needed for child care staff on health topics, <br />including such topics as diapering, hand washing, immunizations, asthma and allergies, <br />and food safety <br />Specific outcomes anticipated as a result of this project include: <br />• 90% of child care providers participating in trainings will demonstrate increased <br />knowledge on post-test scores <br />• 95% of children's health records reviewed by the consultant will show that the child has <br />up-to-date immunizations within 30 days of enrollment at the center <br />• 60% of children enrolled at sites that are referred for early intervention of health services <br />will receive services within 90 days of referral (service acceptance is voluntary) <br />This program fits well into the continuum of current services offered to children by the Health <br />Department. The Health Department currently offers child service coordination for children with <br />developmental delays for ages 0-5; intensive home visiting services for young and first time <br />mothers with high-risk factors with infants 0-3 months; well-child clinical services and as of <br />,January 3; primary care services for children up to age 21; screening of birth records for high- <br />risk factors and referral to appropriate services; and coordination and consultation for school <br />nursing services in both districts for school-age children. Many health departments across <br />North Carolina provide this service to child care centers in their counties, often through local <br />Smart Start partnership funding. <br />The Health Department is proposing adding one Permanent Full Time Public Health Nurse <br />position to carry out the project as described in the grant application. The position would be <br />authorized contingent upon receipt and continuation of non-county funds. <br />The OCPYC Board has signaled its commitment to the Child Health Care Consultant program <br />by specifically recruiting for this type of service outside of its usual proposal cycle and <br />identifying this program and the Health Department as among those eligible for multiple year <br />funding based on previous success in achieving high program standards and outcomes. While <br />funding levels may change, it is projected that health consultants in child care settings will <br />remain a top Partnership priority. As part of the ongoing monitoring of grantees, an extensive <br />annual evaluation is prepared for the OCPYC. The Board of Commissioners will receive a copy <br />of the annual evaluation that tracks project outcomes. <br />FINANCIAL IMPACT: The award from the OCPYC for the remainder of the 04-05 fiscal year is <br />$23,570 and the anticipated award (subject to State budget allocations) for 05-06 is $55,000. <br />This amount is adequate to cover the salary and benefits of a Public Health Nurse II and <br />provide travel and operational costs. <br />The Health Department would be contributing in-kind, nursing supervisory time for the position. <br />The following table reviews the anticipated breakdown of expenditures for the entire 18 month <br />period.
