Of the children enrolled in participating child care sites who are referred <br />for early intervention or health services, 60% will receive services within 90 days of referral. Of the <br />child care providers participating in trainings, 90% will demonstrate increased knowledge on post-test <br />scores. In FY 05-06, 95% of health records reviewed will show up-to-date immunizations within 30 <br />days of enrollment. <br />Outputs: This project will offer services to -120 licensed child care centers/homes in Orange County, <br />of which it is anticipated that at least 50 (50%) will participate in at least one service offered by the <br />child care health consultant. Staff from at least 30 sites (1/4 of the 120 licensed sites) will receive <br />technical assistance to improve environmental ratings related to health and safety. The nurse will <br />screen -250 health/immunization records (-10 sites likely to request immunization review). <br />Evaluation: Child care center/home staff will be surveyed annually regarding their satisfaction of the <br />services offered. They will also be given the opportunity to indicate areas of assistance they may need <br />that has not been available. The Child Care Health Consultant will create tracking logs (for phone <br />calls, site visits, records reviewed, Emergency Medical Plans developed) and maintain training <br />attendance rosters to track the following data: <br />? # of consultation phone calls from child care centers\homes requesting assistance and the type <br />of assistance requested. <br />? # of field visits to child care centers and homes and reason for site visit.. <br />? # of health & immunization records reviewed (immunization compliance, medical homes, <br />insurance enrollment). <br />? # of children referred for early intervention/preschool services <br />? # of emergency medical plans reviewed and # of consultation services provided. <br />? # of children referred for care compared to timeliness and outcome of follow-up/treatment. <br />? # of child care center/home staff and families who receive health education training. <br />? % of demonstrated knowledge gained by participants in trainings (compare pre- and post-test <br />scores of attendees). <br />Consumer Involvement and Satisfaction: Participating child care providers will be surveyed <br />annually using a customer satisfaction survey approved by OCPYC. Providers will be asked to give <br />input on the effectiveness of services they are utilizing. The information gathered will be documented <br />in each Annual Report to OCPYC and used to guide ongoing programmatic improvements. <br />Budget Narrative: We are requesting Smart Start Funds in the amount of $21,680 for the period of <br />January 1- June 30, 2005, and $55,000 for July 1, 2005 - June 30, 2006 to cover salary and fringes for <br />one FTE Public Health Nurse II (Registered Nurse) with experience in pediatrics and community <br />settings as well as operating expenses such as phone, travel and postage. Note per the timeline of <br />hiring a nurse, this salary reflects 4.5 months of project service. The second year salary captures the <br />anticipated 2% cost of living raise anticipated for Orange County employees. Smart Start funds are <br />needed for this project because the services offered by a Child Care Health Consultant are not billable <br />services and are not supported through other funding sources at this time. Therefore, there are no other <br />federal or state dollars funding a project such as this in our agency. Since the starting minimum salary <br />for a Public Health Nurse H with the Orange County Health Department is $42, 954 and the average <br />cost for fringes is 15% of the salary, there will not be ample money left over to cover other expenses <br />such as mileage reimbursement and training. We are requesting $1,890 to help offset the costs of <br />training and materials for the six months (January 2005- June 2005) of the project. The Orange <br />County Health Department will provide $3,000 in matching funds for the first 6-month period and <br />$6,000 in matching funds for July 1, 2005 - June 30, 2006 as in kind costs for nursing supervision, <br />associated with the position. Other support needed will be in the form of guidance from the Orange <br />County Partnership for Young Children because from their ten-year history in funding this position as <br />4
