This proposal covers labor for sample collection, data analysis, laboratory fees for measuring <br />airborne fungi and surface fungi, and report writing. <br />Based on our telephone conversation we propose the following tasks: <br />1. Visual inspection of the impacted area. <br />2. Record Indoor Air Quality parameters of temperature and relative humidity in the structure. <br />3. Check building materials moisture content in representative suspect areas. <br />4. Collect representative fungal spore trap air screening samples indoors and outdoors for comparison for non - viable fungal analysis. <br />At least one of the samples indoors will be collected at the return with the fan of the HVAC operating. <br />5. Collect representative surface sample(s) for non - viable fungal analysis. <br />6. The proposed cost is an estimate based on our telephone <br />conversation. If the investigation is more extensive and involving more areas, the price will be adjusted but will be discussed with you while we are on site. We request <br />payment of 60% of the total upon completion of our investigation and the remainder on submission of our report. We accept Visa, MasterCard or a check at the time <br />of our investigation. If the proposal is acceptable, please sign and return a copy to serve as our notice to proceed. An email confirmation is acceptable. Thank you for <br />the opportunity to work on this project. <br />After the completion of remediation, clearance testing is highly recommended.
