Primary data will be collected from community members, clients receiving direct <br />services from the health department, the Board of Health and other county <br />stakeholders through their participation in written surveys, focus groups, or <br />interviews. <br /> <br />f. Secondary data will be obtained from the North Carolina State Center for Health <br />Statistics (NC SCHS) and other resources identified and available to CHA team <br />members. <br /> <br />g. CHA team members will assist with the collection of primary and secondary data. <br /> <br />h. The CHA team will collaborate with county stakeholders to prioritize health <br />concerns according to the primary and secondary data collected. The CHA team <br />will reference the CHA Guidebook for guidance on reporting data findings and <br />involving community members, the Board of Health and other county <br />stakeholders in the process to establish health priorities for the county. <br /> <br />i. Designated members of the CHA team will summarize the data and priority health <br />topics to produce a document to report the community health assessment process <br />and its findings. <br /> <br />j. The CHA document will be submitted to the North Carolina Division of Public <br />Health by the first Monday in March every four years and will be disseminated to <br />community and county stakeholders as specified in the North Carolina Local <br />Health Department Accreditation Standards. This may include electronically via <br />the department’s website as well as presentations of findings and copies of reports <br />to partner agencies and community organizations for public access. <br /> <br />k. The CHA document will be used by the Orange County Health Department in the <br />development of the department-wide strategic plan, grant writing, program <br />planning and advocacy for funding. This document will be available as a resource <br />for other individuals, agencies, and organizations. <br /> <br />1. Using the CHA, Health Promotion and Education staff and the CHA team will <br />create Community Health Action Plans to describe plans for health activities to be <br />carried out in the county. The Community Health Action Plan form is due the <br />first Monday of September the year the county was assigned to complete their <br />CHA. The form is available through the NC Division of Public Health website.
