MAIN APPLICATION <br />Activities take place at community center -like locations. Past sites have included the Chapel Hill <br />Public Library, the Unitarian Universalist Church, and the Cedar Ridge Parks Department. <br />f) Describe specifically the period over which the activities will be carried out, the frequency <br />with which the activities will be carried out, and the frequency with which services will be <br />delivered. Include an implementation timeline. <br />August 2016 Begin preparation for programming (buying supplies, getting in touch with <br />schools, writing MOAs, etc.) <br />January 2017 Set up dates /time for spring health fair, workshops, other activities <br />February 2017 Begin recruitment process through guidance counselors, social workers, and <br />referrals for Smart Girls and Teen Voices <br />March 2017 Teen Voices and Smart Girls start <br />• Teen Voices — 12 weeks, 12 sessions <br />• Smart Girls — 10 weeks, 10 sessions <br />March -April 2017 Parent session in between the program dates for both Teen Voices and Smart <br />Girls (Other parents are allowed to attend) <br />May /June 2017 Graduation for Smart Girls and Teen Voices <br />Throughout the year - workshops, health fairs, Teen Voices Alumni meets once a month <br />g) Provide a bulleted list of other agencies, if any, with which your agency <br />coordinates /collaborates to accomplish or enhance the Projected Results in the Program(s) <br />to be funded. For each, give specific examples of the coordinated /collaborative efforts. <br />• Compass Center — participant recruitment <br />• Orange County Library Systems — hosting workshops, sessions, and alumni meetings <br />• Chapel Hill, Carrboro, and Orange County Schools — participant recruitment <br />• El Centro Hispano — participant recruitment <br />h) Describe what would happen if requested funding is not awarded at all or if a reduced <br />allocation is recommended. <br />PPSAT is committed to maintaining these education programs in Orange County. In addition to <br />County and Town support, PPSAT receives funding for its educational programs from a number of <br />private foundations. All of these foundations have a long history of supporting our programs and we <br />anticipate their renewed support for FYI 6.
