It uses the most recent data to highlight the <br />leading Orange County (OC) causes of death and disease and progress towards addressing the <br />leading health concerns identified in the 2015 Community Health Assessment (CHA). The <br />prioritized health issues are: 1) Social Determinants of Health with priority around access, <br />transportation and poverty; 2) Physical Activity and Nutrition with priority around chronic disease <br />prevention, and 3) Mental Health and Substance Abuse. <br /> <br />This report will educate and inform community members, community leaders, and organizations <br />about Orange County’s progress on these chosen priority health issues. This report will be <br />available electronically on the Healthy Carolinians of Orange County and Health Department’s <br />website, and paper copies will be available at the Health Department. The Orange County <br />Health Department will distribute this report to the Orange County Board of Health, Orange <br />County Board of County Commissioners, the Healthy Carolinians of Orange County <br />membership, Orange County Government employees, and others upon request. <br /> <br />Financial Considerations: None <br /> <br />Recommended Motion (if any): <br />
