Attachment - 2 <br /> <br /> <br />• Attempting to access a secured application or restricted area without proper <br />authorization or for purposes other than official UNC Health Care business. <br /> <br />I understand the UNC Health Care: <br /> <br />* Employs security and auditing technologies to track and log my activity <br />within the UNC EMR system including all patient records I have viewed; <br />* Produces reports and requires audits of my patient records accesses; <br />* Requires my employer to take disciplinary action if I commit an <br />inappropriate patient access <br />* May terminate access of ALL of my employer’s UNC users if I commit an <br />inappropriate patient access <br /> <br />Violation of this statement may constitute grounds for corrective action up to and including <br />termination of employment, loss of UNC Health Care System privileges or contractual or <br />affiliation rights in accordance with applicable UNC Health Care System procedures. <br />Unauthorized use or release of confidential information also may subject the violator to personal, <br />civil, and/or criminal liability and legal penalties. <br /> <br />I have read and agree to comply with the terms of the above statement and will read and comply <br />with the Health Care Privacy and Confidentiality of Individually Identifiable Health Information <br />(Protected Health Information or PHI) and Information Security Policies, as applicable, copies of <br />which will be provided upon request. <br /> <br /> <br />EXAMPLES OF BREACHES OF CONFIDENTIALITY <br /> <br />Accessing confidential information that is not <br />within the scope of your duties: <br /> <br />Unauthorized reading of patient account <br />information; <br /> <br />Unauthorized reading of a patient’s chart; <br /> <br />Unauthorized access of personnel file <br />information; <br /> <br />Accessing information that you do not “need-to- <br />know” for the proper execution of your duties. <br /> <br />Misusing, disclosing without proper <br />authorization, or altering confidential <br />information: <br /> <br />Making unauthorized marks on a patient’s <br />chart; <br /> <br />Making unauthorized changes to a personnel <br />file; <br /> <br />Sharing or reproducing information in a <br />patient chart or a personnel file with <br />unauthorized personnel; <br /> <br />Discussing confidential information in a <br />30
