a. "Fit Feet" Clinic - funds received totaling $2,500 from participants in the "Fit Feet" <br />clinic program. This budget amendment provides for the receipt of these funds. (See <br />Attachment 3, Senior Health Coordination Grant Project Ordinance) <br />b. Blue Cross/Blue Shield Mini-Grant for "Fit Feet" - receipt of grant funds totaling <br />$5,700 between Blue Cross and Blue Shield of North Carolina and the Friends of the <br />Chapel Hill Senior Center on behalf of the County's Fit Feet wellness program. This <br />program is a basic foot health care assessment program, where registered nurses <br />assess clients who are 55 years and over, and are at risk of health problems due to <br />untreated wounds, ulcers, or sores associated with the feet. No County match is <br />required for the receipt of this grant. This budget amendment provides for the receipt <br />of these grant funds. (See Attachment 3, Senior Health Coordination Grant Project <br />Ordinance) <br />c. Partners Medicaid Choice Walking Program - receipt of grant funds totaling $2,500 <br />from Partners Choice for sponsorship in a Wellness Mall Walking Program. This is a <br />joint venture between University Mall in Chapel Hill, the Cop Shop - a division of the <br />community policing for the Chapel Hill Police Department located within the mall, and <br />the Wellness Program of Orange County. Funds will be used to develop a daily <br />walking program with incentives for walkers and a monthly healthy breakfast with a <br />health and wellness related speaker for ten different months a year. No County <br />match is required for the receipt of this grant. This budget amendment provides for <br />the receipt of these grant funds. (See Attachment 3, Senior Health Coordination <br />Grant Project Ordinance) <br />d. United Way funds - receipt of additional United Way funds totaling $8,730 awarded <br />through the Friends of the Chapel Hill Senior Center for the wellness program. This <br />budget amendment provides for the receipt of these additional funds. (See <br />Attachment 3, Senior Health Coordination Grant Project Ordinance) <br />Miscellaneous <br />On June 30 of each year, certain designated appropriations are not fully expended. Many of <br />these relate to outstanding purchase orders where goods and services have been ordered <br />but not yet received and/or paid. In accordance with the annual budget ordinance, funds for <br />these outstanding purchase orders are automatically re-appropriated in the new fiscal year's <br />budget. On the other hand, there are instances where approved funding for certain <br />initiatives is not fully expended at the end of the fiscal year, Funds for these planned, but <br />unencumbered expenditures, must be re-appropriated from the County's fund balance. An <br />example of this includes unexpended monies from the Social Safety Net totaling $150,952. <br />In order to carry these funds forward and authorize their expenditure in fiscal year 2004-05, <br />it is necessary for the Board to approve the attached amendment. (See Attachment 1, <br />column #6) <br />Environment and Resource Conservation <br />On December 14, 2004, the Board of County Commissioners approved the acceptance of <br />an agricultural and watershed protection easement from John and Carolyn Lloyd, which will
