Scorecard Instructions <br />Using the Application Scorecard, pleases assess the five categories of each agency's application <br />(Mission, Collaboration, Customer, Results, and Plan). <br />Each category contains evaluative components (column A), with references (column B) on where to <br />find the responses in the application. Then, determine whether the agency responded to the <br />question and place an "X" in the appropriate box (columns C or D). If the agency did answer the <br />question to some degree, award an appropriate number of points (column F) out of the total <br />number of points possible for that question (column E). <br />X <br />Advisory Boards should use the Rescaled Scorecard for Agencies that are NEW and lacking FY 2016- <br />17 program results (Section 3 - O Work Statement). The Rescaled Scorecard will calculate that <br />agency's total score out of 75 rather than 100. <br />Microsoft Excel will calculate any entries in Column E for each category, subtotal and the grand total. <br />If completing the form by hand, please calculate the totals manually and enter amounts in the <br />appropriate fields. <br />Return the completed scorecards in Microsoft Excel format and comments to Allen Coleman at <br />acoleman@orangecountync.gov, by Thursday, March 29, 2018. <br />Submit ONLY one scorecard per outside agency.
