surface soils investigation, evaluation report, and foundation recommendation for the <br />proposed wireless support structure prior to the commencement of land disturbing <br />activities associated with the construction of the telecommunication facilities. <br />7. For public safety as required in 5.3.2(B)(2), final assignment of a street address shall be <br />completed by Orange County Land Records prior to the issuance of any permit <br />authorizing land disturbing activity on the property in accordance with the Orange County <br />Addressing Ordinance. <br />8. Per Section 5.10.8(B)(4)(j) and Section 2.5 of the UDO the applicant shall obtain all <br />necessary development permits from the County prior to the initiation of and land <br />disturbing activity associated with the construction of the telecommunication facilities <br />including, but not limited to: <br />a. Building Permit, <br />b. Erosion Control Management Permit, <br />c. Solid Waste Management Permit, and <br />d. Zoning Compliance Permit. <br />9. Any proposed co-location of antenna on this tower shall be reviewed, acted upon, and <br />installed in accordance with the provisions of Section 5.10.8 (A) of the UDO. <br />10. Per Section 5.10.8 (B) (5) and (6) of the UDO the applicant shall submit all necessary <br />bonding/financial security documents to the County Attorney’s office for review and <br />approval guaranteeing the removal of the tower in the event it is abandoned or unused <br />for a period of 12 months. A cost estimate shall be provided by a qualified contractor. <br />The amount of the security shall be 110 percent of the estimate. This must be completed <br />before building permits are issued. <br />11. Per Section 5.10.8(B)(4)(j) and Section 5.10.8 (C) of the UDO telecommunication tower <br />owners shall submit a report to the County Inspections Division certifying structural and <br />electrical integrity upon completion of the initial construction and at intervals as specified <br />within the UDO. <br />12. Per Section 5.10.8(B)(4)(j) and Section 5.10.8 (C) (1) (b) of the UDO inspection records <br />shall be kept by the tower owner and made available upon request to the County <br />Inspections Division during regular business hours. Inspections shall be performed as <br />specified within the UDO. <br />13. Per Section 5.10.8(B)(4)(j) and Section 5.10.8 (C) in those cases where an inspection is <br />required, which is not performed by Orange County Inspections, the applicant is required <br />to notify the Planning Department and any applicable County telecommunication <br />consultant of the inspection and its results. <br />14. Per Section 5.10.8(B)(4)(j) commercial service cannot be provided/initiated until the <br />County’s telecommunications consultant issues a final Certificate of Completion upon the <br />completion of a final inspection of the constructed telecommunication facilities as <br />detailed in 5.10.8 inclusive of the UDO have been adhered to. <br />15. Per Section 5.10.8(B)(4)(j) and Section 2.7.12 (D) of the UDO the Special Use Permit will <br />automatically expire within 12 months from the date of approval if the use has not <br />commenced or construction has not commenced or proceeded unless a timely <br />application for extension of this time limit is approved by the Board of Adjustment. <br />16. Per Section 5.10.8(B)(4)(j) and Section 2.7.12 (C) of the UDO if any condition of this <br />Special Use Permit shall be held invalid or void, then this Special Use Permit shall be <br />163
