No Fault Well Repair Fund Memo <br /> <br />A copy of the follow-up memo requested by the Commissioners providing an update to the <br />No Fault Well Repair Fund by Tom Konsler, Environmental Health Services Director. <br /> <br />d. Restaurant Heart Health Statewide Survey Report <br /> <br />Board members were provided a copy of the Restaurant Heart Health Statewide Survey. <br />Orange County participated in this survey in 2006-07 and will participate again in 2008-09. <br /> <br />e. Letter to Commissioners Requesting Meeting <br /> <br />A letter requesting a joint meeting between the Board of Commissioners and the Board of <br />Health in the fall was sent by Board of Health Chair, Matt Vizithum in June. The Board is <br />waiting a reply by the Commissioners. <br /> <br />f. Media Information Items <br /> <br />Various news articles about Health Department programs or issues the department is <br />involved in were provided for the Board. <br /> <br />F. Tracking Form Review <br /> <br />The Tracking Form had no changes and was provided to give the Board awareness of upcoming <br />agenda items. <br /> <br />G. Dental Fees <br /> <br />Dental Health Services requested the addition of a dental service charge for the fabrication of an <br />occlusal guard that is designed to minimize the effects of bruxism (grinding) and other occlusal <br />factors. The average fee for this service is $274.00. The lab cost to fabricate the appliance is <br />$95.00. The dental patient would be responsible for the full cost of the lab charge in addition to <br />the fee of $274.00 at their percent level on the sliding scale. <br /> <br />ADA Code: D9940 Occlusal guard, by report <br /> <br />Dental Health Services requested the increase in the 100% fee for one dental service due to the <br />increase in the Dental Medicaid rates effective 2007/08. The rate was increased based on the <br />Fee and Eligibility Policy. <br /> <br />ADA Code: D7310 Alveoplasty in conjunction with extractions – four or more teeth or tooth <br />spaces, per quadrant. <br />
