Contract renewal with UNC Family Practice for Medical Director

OCHD has maintained a contract with UNC Family Practice for Medical Director Services for a
number of years. Dr. Michael Fisher has served in this capacity for the past five years with back
up from Dr. Lauren Lingley and other physicians. Dr. Fisher is well received by clients and staff
enjoy working with him.

The 07-08 contract expands the number of hours from 28 to 30 hours per week in recognition of
additional primary care services and an increase in clinical and administrative duties.

In addition, Chapel Hill Carrboro Schools (CHCCS) is seeking renewal of its contract with the
Medical Director for medical supervision of one of its School Nurses who is a qualified Nurse
Practitioner to provide limited Nurse Practitioner services in the Head Start Program.

C. Contract renewal with Robert DuPuis for Pharmacy Services

The issuance of pharmaceuticals through our clinics or during epidemics/disasters necessitates
the services of a registered pharmacist. Robert Dupuis, Pharm.D., a Clinical Associate Professor
in the Division of Pharmacy Practice at UNC School of Pharmacy, has served as our Pharmacist
the past five years. He has fulfilled his responsibilities well and is interested in continuing to
serve in this capacity.

Changes in the 07-08 Pharmacy Contract include provision of medications for low-income
clients through the Medication Assistance Program, personal liability insurance amounts,
independent Contractor language, and a 3.5% increase to $11,628 per year. This is still well
below the market rate for pharmacy services.
