4 <br />Healthcare Access Project (HCAP). The department plans to collect data from uninsured <br />patients for whom they provide services, ~ and provide the information/statistical data to <br />AccessCare/CCHN for evaluation. The State will reimburse, not to exceed $13,750, the <br />Health Department for operating costs associated with the AccessCare/CCHN case <br />manager at the Health Department. This budget amendment provides for the receipt of <br />these reimbursable funds for the above stated purpose. (Attachment 1, column #9) <br />Human Services <br />10. Currently budgeted Home and Community Care Block Grant (HCCBG) funds total $408,977. <br />The County. has received notification of an additional allocation of HCCBG funds totaling <br />$29,372. The addition of these funds brings the current year's total HCCBG allocation to <br />$438,349. Based on an evaluation committee's recommendation, staff recommends that <br />Commissioners allocate the additional funds as follows: <br />a. Social Services - $15,972 to offer in-home services for currently under served <br />individuals who are at risk of abuse, neglect or out-of-home placement <br />• .Aging - $10,000 to provide outreach/education and community referral/support <br />services for African Americans at risk for dementia <br />• JOCCA - $3,400 to offset operational costs of providing services <br />It is important to note that while Orange County's HCCBG appropriation totals $438,349, the <br />$3,400 allocation for JOCCA will go directly to the agency and will not flow through the <br />County's budget, therefore not included in this budget amendment. This budget amendment <br />provides for approval of the amended HCCBG Funding Plan and appropriates the County <br />departments' share of the amended appropriation ($15,972 for DSS and $10,000 for Aging). <br />(Attachment 6, HCCBG Funding Plan). <br />FINANCIAL IMPACT: Financial impacts are included in the background information above. <br />RECOMMENDATION (S): The Manager recommends that the Board of County Commissioners <br />approve the attached budget ordinance amendments and capital project ordinances. <br />
