volunteer seniors would call isolated seniors on a regular basis. The first goal under the Care <br />Management subgroup involves getting county agencies to meet and share information and <br />concerns regarding common clients and services. A lot more can be done by working together.. The <br />second goal involves coordinating services among private, public and volunteer agencies. <br />Kate Barrett said that she has a lot of questions about how this can be done. This seems to be <br />something that people are interested in but the question of who would be responsible world need to <br />be addressed. The third goal would be to develop a voluntary database of clients so that crucial <br />information regarding clients will be available to professionals. The fourth goal would be to add Co <br />the Orange County Resource Guide website state and national professional organizations and the <br />last goal is to add professional staff capable of continuing physical rehabilitation interventions for <br />seniors discharged from home health rehabilitation but who are still in need of follow-up in order to <br />maintain optimal functioning. The Mental Health subgroup is proposing to have a partnership and <br />implement a depression care system. A <br />depression care specialisC would be in the physician's offiice. There would be a geriatric psychiatrist <br />who would serve in a consulting role. What you do is to remove the stigma of going to a Mental <br />Health facility. This would also reduce the cost, It would take the Department on Aging working <br />with UNC to make this work. The last subgroup working with dementia would include a massive <br />outreach to minority churches. This would include information on how to get help. <br />Ann .Johnson said that she feels it is important for people who are incontinence to <br />realize that they don't have Co be incontinent, necessarily. <br />Jerry Passmore said that he plans to meet with the architect for the new senior center <br />and he will certainly make this point.. <br />C. Institutionalized/Severely Impaired Functional Group- Jill Passmore <br />.lilt Passmore said that they have identified Four (4) issues for the severely <br />impaired/institutionalized older adults. They are listed below: <br />• Quality of Life and Care issues <br />•:• Caregiver Support/Work Force Issues <br />• Education/Outreach <br />•'• Person-directed Care <br />They tools the previous Master Aging Plan and reviewed it and the priorities that were assigned and <br />looked at all the objectives that were previously assigned to these priorities. They then decided to <br />either keep as is or amend or delete them. The last meeting was spent on this project.. They will <br />take this information back to the committee and look at where there are overlaps and holes in the <br />plan. They will then set an action plan.
