<br />THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY.
<br />This endorsement modifies insurance provided under the following:
<br />COMMERCIAL GENERAL LIABILITY COVERAGE PART
<br />With respect to coverage provided by this endorsement, the provisions of the Coverage Form apply unless modified by the
<br />endorsement.
<br />1. BLANKET ADDITIONAL INSUREDS
<br />a. Ongoing Operations
<br />SECTION II — WHO IS AN INSURED is amend-
<br />ed to include as an additional insured any person
<br />or organization you have agreed in a written
<br />contract, written agreement, or written permit to
<br />add as an additional insured on your policy, but
<br />only with respect to liability arising out of your
<br />ongoing operations performed under that con-
<br />tract, agreement, or permit when that contract,
<br />agreement, or permit requires the additional
<br />insured be added with respect to liability arising
<br />out of your ongoing operations. If the written
<br />contract, written agreement, or written permit
<br />does not require that the additional insured be
<br />added with respect to liability arising out of your
<br />ongoing operations, then SECTION II — WHO
<br />IS AN INSURED is amended to include as an
<br />additional insured any person or organization
<br />you have agreed in a written contract, written
<br />agreement, or written permit to add as an addi-
<br />tional insured on your policy, but only with re-
<br />spect to "bodily injury', "property damage" or
<br />"personal and advertising injury' caused in whole
<br />or in part by your ongoing operations performed
<br />under that contract, agreement, or permit.
<br />b. Completed Operations
<br />SECTION II — WHO IS AN INSURED is
<br />amended to include as an additional insured any
<br />person or organization you have agreed in a
<br />written contract, written agreement, or written
<br />permit to add as an additional insured on your
<br />policy, but only with respect to their liability
<br />arising out of "your work" performed under that
<br />contract, agreement, or permit and included in
<br />the "products- completed operations hazard"
<br />when that contract, agreement, or permit
<br />requires the additional insured be added with
<br />respect to liability arising out of "your work"
<br />performed under that contract, agreement, or
<br />permit and included in the "products - completed
<br />operations hazard ". If the written contract, written
<br />agreement, or written permit does not require
<br />that the additional insured be added with respect
<br />to liability arising out of "your work" performed
<br />under that contract, agreement, or permit and
<br />included in the "products- completed operations
<br />hazard ", then SECTION II — WHO IS AN
<br />INSURED is amended to include as an addi-
<br />tional insured any person or organization you
<br />have agreed in a written contract, written agree-
<br />ment, or written permit to add as an additional
<br />insured on your policy, but only with respect to
<br />liability for "bodily injury ", "property damage" or
<br />"personal and advertising injury" caused, in
<br />whole or in part, by "your work" performed under
<br />that contract, agreement, or permit and included
<br />in the "products- completed operations hazard ".
<br />c. The coverages provided in Paragraphs a, and b.
<br />do not apply unless the written contract or written
<br />agreement has been executed (executed means
<br />signed by the named insured) or written permit
<br />issued prior to the "bodily injury', "property
<br />damage" or "personal and advertising injury'.
<br />d. Exclusions
<br />(1) With respect to the insurance afforded to
<br />additional insureds under a. Ongoing Oper-
<br />ations the following is added to 2. Exclu-
<br />sions under SECTION I — COVERAGE
<br />A— BODILY INJURY AND PROPERTY
<br />DAMAGE LIABILITY:
<br />This insurance does not apply to "bodily
<br />injury", "property damage ", or "personal and
<br />advertising injury" occurring after:
<br />(a) All work, including materials, parts or
<br />equipment furnished in connection with
<br />such work, on the project (other than
<br />service, maintenance or repairs) to be
<br />performed by or on behalf of the addi-
<br />tional insured(s) at the site of the cov-
<br />ered operations has been completed; or
