Rent assistance <br />The Contractor may only spend funds in categories that were applied for and awarded in the NC <br />ESG application that was submitted to the State prior to the contracting period January 1, 2018 - <br />December 31, 2018. <br />j. Spending Rates <br />Contractors must expend ESG fiends on monthly basis until the contracted amount is expended on or <br />before December 31, 2018. Additionally, contractors must have at least one requisition leading to a <br />disbursement of ESG funds per quarter. <br />k. Monitoring <br />The Contractor will participate in all monitoring activities, programmatic and /or fiscal, state and /or <br />federal, field and /or desk monitoring (materials mailed /emailed in to the state offices). <br />a. At a minimum, participant file monitoring will include review of households that were not <br />enrolled into the program, households that are currently enrolled in the program, and <br />households that have exited the program (unless no households have yet exited) <br />i. Fiscal monitoring will include monitoring of fiscal processes, financial <br />documentation of expenses, and eligibility documentation in client files. <br />ii. Programmatic monitoring will also include review of HMIS data entry and <br />monitoring practices. <br />iii. For Housing Stability activities: Participant file monitoring will include, but is not <br />limited to, eligibility determination and recertification, Housing Stabilization Action <br />Plans, financial assistance determination, financial assistance documentation, <br />referral and follow up documentation, and HMIS consent forms. <br />iv. hispections: Shelter and Housing inspections must meet HUD's minimum <br />emergency shelter and permanent housing standards. <br />Documentation: Monitoring reports and participant files <br />1. Reporting <br />Contractors will be responsible for accurate and complete statewide EMS data entry (comparable <br />data for Domestic Violence programs) and submission of any state, federal, local HMIS <br />administrators' request. <br />All HMIS data entry for the quarter must be entered into the statewide system by the end of the third <br />month of the quarter. <br />Outcomes (06/04) Page 3 of 6 <br />
