Pdf -A (Archival format) is preferred but <br />a high- quality standard PDF file is also acceptable. <br />b. Site and Accession Numbers <br />All reports of archaeological investigations must use permanent archaeological site numbers <br />issued by the OSA. Cemeteries within a project area that have interments 50 years old or <br />older should also have a permanent site number assigned. Reports that discuss <br />archaeological sites and /or cemeteries will not be accepted without permanent site numbers. <br />Site number assignments will be made in response to letter or email requests to the Site <br />Registrar. Requests should be accompanied by detailed maps showing accurate site locations <br />and project boundaries, with USGS quadrangle names noted on each. Site - specific <br />information should include, at a minimum, whether a site was occupied in prehistory, during <br />the historic era, or at some point during both periods. Submission of shapefiles along with <br />site maps is encouraged. An example of the preferred format for the submission of <br />permanent site number requests is provided in Appendix B. <br />If artifacts are to be curated at the Office of State Archaeology Research Center ( OSARC), <br />accession numbers should be obtained from the Site Registrar when site numbers are <br />requested and assigned. Curation standards for material to be curated by the OSARC are <br />included as Part 4 in this document. <br />Please be advised that it may take up to five business days to fulfill site and accession <br />number requests. No permanent site numbers will be assigned on the basis of informal <br />contacts such as telephone calls. No single numbers or blocks of numbers will be assigned in <br />advance of field investigations or in anticipation of survey results. <br />c. Site and Cemetery Forms <br />All newly identified or revisited archaeological sites and cemeteries documented in a report <br />of field investigations should be recorded using the appropriate OSA forms. Reports must <br />be accompanied by the appropriate forms to be considered complete and sufficient for <br />review. <br />Archaeological sites should be recorded using the North Carolina Archaeological Site Form <br />version VIII. This form is located in Appendix C and is also available on the OSA web site <br />(http: / /archaeology.ncdcr.gov /programs /forms). Earlier versions of this form (including <br />Site Form III) will no longer be accepted. Do not change the format, font size, or font type <br />of the site form. <br />Cemeteries containing interments greater than 50 years of age should receive a trinomial site <br />number. If the cemetery is associated with other historic site elements, or is located within a <br />prehistoric site, both a completed OSA site form and a cemetery form should be submitted. <br />Otherwise only a cemetery form is necessary. This form is located in Appendix D and is also <br />available on the OSA web site (http: / /archaeology.ncdcr.gov /programs /forms). Given the <br />possibility for unmarked graves in historic cemeteries, even burial locations with a single <br />above - ground marker should be recorded as cemeteries. See Part 2, Terrestrial Field <br />Methodology, for guidelines concerning cemetery documentation.
