DocuSign Envelope ID: BAEC321D -F111- 4756- 8F3E- C94BOFAC92A6 <br />NORTH CAROLINA CONTRACT AMENDMENT <br />ORANGE COUNTY <br />THIS CONTRACT AMENDMENT ( "Amendment ") is made and entered into this I" day of February, 2018 by and between <br />ORANGE COUNTY (hereinafter referred to as "County ") and NYMIA Group, Inc. (hereinafter referred to as "Consultant "). <br />WITNESSETH: <br />THAT WHEREAS, the County and Consultant entered into a contract dated November 21, 2017, (hereinafter the "Original <br />Agreement'), for the provision of consulting services for the refraining of housing program offerings to the County; and <br />WHEREAS, the County and Consultant desire to amend the Original Agreement while keeping in effect all terms and conditions of the <br />Original Agreement not inconsistent with the terms and conditions set forth below. <br />NOW THEREFORE, for and in consideration of the mutual covenants and agreements made herein, the parties agree to amend the <br />Original Agreement as follows: <br />In order to ensure the completion of the Services identified in the term of the Original Agreement is amended to reflect an <br />end date by which all Services shall be completed of June 30, 2018. <br />Exhibit 1 to the Original Agreement is amended by adding the following tasks and services to the Services to be provided <br />by the Consultant: <br />• Consultant will oversee the correction of staff PIC errors; <br />• Consultant will oversee the development and initial implementation of a corrective action plan per US HUD findings for <br />the 2017 SEMAP, as well as prepare department for the 2018 SEMAP <br />• Consultant will revise program processes and procedures and create operational standards manual; <br />• Consultant will provide training under approved training plan and training guides on all approved trainings; <br />• Consultant will participate in hearings related to the HVC program and provide follow up support when appropriate; <br />• Consultant will compile a final report — Phase II to present for review /approval on Orange County's HCV program; <br />• Consultant will provide technical assistance and training for general housing and community development staff when <br />appropriate; <br />• Consultant will assist with the planning and coordination of two (2) events, one for developers and one for mobile home <br />park owners. Both events are being held to enhance the entities participation in Orange County affordable housing <br />programs. <br />• Consultant will refine existing housing policies and propose new polices that will lead to increasing affordable housing <br />options in Orange County; and <br />• Consultant will submit a final summary report on all services and activities. <br />Article 5, Section 5. 1.1 is amended to reflect a maximum payable not -to- exceed amount of $72,850. <br />4. Except for the changes made herein, the Original Agreement shall remain in full force and effect to the extent it is not <br />inconsistent with this Amendment.
