Program Activity Name <br />Club Nova offers the spectrum of services to ACTT members regardless of <br />our ability to receive payment for these services, due to ACTT /PSR <br />exclusion, which amounts to roughly $110,000 annually. <br />Club Nova actively collaborates with the ACTT providers to coordinate <br />care and ensure the health and safety support of these members. <br />Program Goal <br />20 current members who receive Assertive Community Treatment Team <br />(ACTT) non - billable services (typically those members that experience the <br />most disabling effects of mental illness) will continue to receive full <br />access to the supports and opportunities provided by Club Nova. <br />Performance Measures <br />Club Nova will: <br />• Monitor our provision of services to ACTT members through calculating <br />monthly participation and attendance hours. <br />• Write monthly service notes on each member receiving ACTT services. <br />Previous Year Program Results <br />100% of Club Nova members who received ACTT services, maintained <br />their Club Nova membership, and received supports and services <br />provided through Club Nova. <br />Current Year Estimated Results <br />100% of Club Nova members who receive ACTT services will maintain <br />their Club Nova membership, and receive supports and services provided <br />through Club Nova. <br />Next Year Projected Results <br />100% of Club Nova members who receive ACTT services will maintain <br />their Club Nova membership, and receive supports and services provided <br />through Club Nova. <br />3.
