We wish to advise that the <br />final plans received at this office are approved. Pursuant to this approval, this office will require you to <br />make the owner/developer aware of the Following requirements before starting any construction. <br />l .. The driveway permits for this subdivision was previously approved. (D-06-30-06-0-1 ). <br />2. Certification of Pavement Conformance (Forms QMS-MV2 and QMS-MVl) must he filled out <br />and returned to the District Office thirty (.30) days prior to the installution of asphalt materials. <br />3. The NPDES Stormwater Permit Compliance Certification and the Verification of Compliance <br />with Environmental Regulations forms must be filled out and returned to the District Office. <br />4 During the construction phase, the owner/developer will contact the Graham District Office before <br />each of the following phases: (A) Drainage, (B) Grading, (C) Placement of base, (D) Paving, and (EJ <br />Seeding.. Failure to notify this office before each phase of construction mayjustify verification <br />certified and signed by a professional engineer. <br />5. A mandatory Preconstruction conference is to be scheduled with this office prior to any <br />construction. Contact Shawn Smith, Transportation Technician III, at.336-570-68.3:3 to <br />scheduled this meeting. <br />6 When working within the State's right of way, the owner/developer will contact the Graham District <br />Office before each of the following phases: (A) Drainage, (B) Grading, (C) Placement of base, (D) <br />Paving, and (E) Seeding, Failure to notify this office before each phase ofconstruction mayjustify <br />verification certified and signed by a professional engineer. <br />P.
