Originally it was designed as an online platform to utilize the <br /> VI-SPDAT,prevention resources, and a community resource database to coordinate <br /> homelessness services and resources.As of Feb 30th, 2015 it has been available through a <br /> training offered to individuals on the 100k Taskforce, and has served as the intake/entry <br /> process for referrals onto the HOME Taskforce.Since it's creation 334 assessments have been <br /> made via OC Connect.224 of those individuals scored high enough on the VI-SPDAT to be <br /> referred onto the 100k Taskforce, prioritized for services according to their VI-SPDAT score, <br /> and connected to resources through the resource database. <br /> As a community, Orange County is moving towards a more connected and collaborative <br /> approach to homeless services, moving beyond assessment that prioritizes services for the most <br /> chronic towards a fully coordinated entry system! IIDa 110171 e st IIIItw iiio en c4 11111°1 Screen <br /> Individuals experiencing domestic violence are best served by DV agencies.To this end we are <br /> updating OC Connect to incorporate 3 separate tests to identify that a client is experiencing <br /> domestic violence. If the client is experiencing domestic violence, OC Connect will assist the <br /> interviewer in making the appropriate referral. <br /> 1 Dive iiii ors IIII uI I IIII I <br /> Individuals who are acutely at risk of homelessness will be guided through a diversion tool that <br /> will help them explore opportunities to sustain their current housing or re-access previous <br /> housing options.The system will explore what funding or resources might be needed and <br /> record the results of attempts towards diversion.
