i4. <br />ATTACHMENT C <br />INTER- AGENCY LONG TERM CARE MANAGEMENT PROCEDURES <br />The Orange County Department on Aging provides Long-Term Care <br />Management to all county residents who are sixty years of age <br />and older. It is the process in which the individual is <br />linked to an array of services appropriate to the assessed <br />needs and based on a plan of care specific to the individual. <br />Long Term Care Management provides ongoing support and <br />follow -up to assure an awareness of changes in status for a <br />prolong period, provide smooth coordination of services and <br />avoid unnecessary duplication. <br />Care Management differs from <br />comprehensive and long term. <br />service or agency specific. <br />case management in that is <br />Whereas, case management is <br />In order to more effectively coordinate client services and <br />ensure continued appropriateness of services, the following <br />inter - agency care management procedures are instituted: <br />1• Update the referral source -- DOA Care Management Team <br />periodically on the situation of the requested service <br />from DOA. <br />2. Do not refer to another service area within the Provider <br />agency without conferring with the referral source - DOA <br />Care Management Team. <br />3. Notify the DOA Care Management Team promptly when a <br />specific Provider service is terminated, giving the <br />date, if possible. <br />4. The DOA Care Management Team should always be notified <br />when there is a problem or change in providing the <br />agency service to the client: whether on the part of <br />the agency or the client. <br />5.
