PROGRAM <br />VEHICLE ASSIGNMENT AGREEMENT <br />This agreement entered into this 1 day of July, 1992 by and <br />between the County of orange, hereinafter referred to as The <br />County and orange - Chatham Comprehensive Health Services, <br />hereinafter referred to as OCCHS. <br />The purpose of this agreement is to outline the conditions <br />under which the The County and OCCHS will cooperatively provide <br />human service transportation to Orange County's elderly, infirmed, <br />and disadvantaged as a participating member of the C.A.T. <br />(Coordinated Agency Transportation) system. <br />SPECIFIC RESPONSIBILITIES OF The Count <br />I. To lease the following identified vehicles or similar <br />size at the hours specified: <br />1984 Dodge Van with lift, 2B7KB33WXEK311123 <br />SPECIFIC RESPONSIBILITIES OF OCCHS <br />1. To provide all gas, proper maintenance, repairs and insurance <br />(liability /collision -- minimum of 900 of replacement cost) <br />on leased vehicles. Proof of insurance and coverage limits <br />shall be submitted to the Aging Director within 10 days of <br />contract approval. <br />2. To operate and schedule the use of the leased vehicle(s) on <br />behalf of human service clients and approved groups, according <br />to the Orange County Transportation Board and approved by the <br />County Commissioners. <br />3. To allow C.A.T. program agencies access (time /ridesharing) to <br />leased vehicles(s) when not scheduled for use at a rate <br />established by the Orange County Transportation Board and <br />approved by the County Commissioners. <br />4. To complete all required vehicle operation reports and submit <br />to the Orange County C.A.T. Supervisor in the Department on <br />Aging. <br />PAGE 1 OF 2 <br />
