ORANGE COUNTY C.A.T. PROGRAM <br />VEHICLE LEASE AGREEMENT <br />This agreement entered into this 1st day of JUL� ^- 19� <br />by and between the ' <br />hereinafter referred to as Th-z Couni--y ; <br />and eda G D <br />hereinafter referred to as cedar Grove pay _!are Cent--gar <br />The purpose of this agreement is to outline the <br />conditions under which the <br />and will cooperatively provide <br />human service transportation to Orange County's elderly, <br />infirmed, and disadvantaged as a participating member of the <br />C.A.T. (Coordinated Agency Transportation) system. <br />SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br />1. To lease the following identified vehicles or similar size <br />at the hours specified: <br />1977 Chevrolet Bus (CAT 13) 18 pass,Ilift- equipped, CPL3273325257 <br />1984 Ford Van (CAT 9) 15 pass., 1FBES31G5EHB92636 <br />2. To provide all insurance, fuel, maintenance repairs and <br />replacement for each leased vehicle. <br />SPECIFIC RESPONSIBILITIES OF <br />1. To reimburse ORANGE COUNTY at the rate of $.35 per mile <br />for a van or _.55 per mile for a minibus, or .75 per mile <br />for a large_bus, due 30 days after billing. <br />2. To operate and schedule the use of the leased vehicle(s) <br />on behalf of human service clients and approved groups, <br />according to the Orange County Transportation Board policy <br />and procedures. <br />3. To park all leased vehicles at the CAT parking lot (300 <br />West Tryon St., Hillsborough) when not in use. <br />4. To complete all required vehicle operation reports and <br />submit to the Orange County C.A.T. Supervisor in the <br />Department on Aging. <br />5. To establish criteria for client eligibility. The criteria <br />is as follows: (1) inability of employee to provide own <br />transportation, (3) Where employee lives and his ability <br />to get to pickup point, and (4) Behavior on vehicle. <br />The term of this agreement shall be from July 1,1992 <br />through June 30, 1993 unless sooner terminated by either <br />party giving 30 days prior written notice to the other party <br />of intention to terminate. <br />IN WITNESS WHEREOF, CQunty of and <br />-Cedar Gro a Da ar C have executed the <br />Agreement of the date first above written. <br />1E B <br />4 3E <br />ly By <br />Date Date <br />FN: CATL D• <br />
