«.4 2 <br />DOA -736 (Revised 5/92) <br />Ok <br />Home and Community Care Block Grant for Older Adults <br />Agreement for the Provision of County -Based Aging Services <br />This Agreement, entered into as of this 29th day of ` June 19 92 <br />and between__ the County of orange b <br />"County") g (hereinafter referred to as the <br />and the Triangle J Council of Government area Agency on Aging, <br />(hereinafter referred to as the "Area Agency "). <br />Witnesseth That: <br />WHEREAS, the Area Agency and and the County agree to the terms and conditions for <br />provision of aging services in connection with activities financed in part by Older <br />Americans Act grant funds, provided to the Area Agency from the United States <br />Department of Health and Human Services through the North Carolina Division of Aging <br />and state appropriations made available to the Area Agency through the North Carolina <br />Division of Aging, as set forth in a) this document, b) the County Funding Plan, as <br />reviewed by the Area Agency and the Division of Aging, c) the Division of Aging Home <br />and Community Care Block Grant Procedures Manual for Community Service Providers, <br />d) the Division of Aging Service Standards Manual, Volumes I through IV, and, e) the <br />Division of Aging Community Service Providers Monitoring Guidelines. <br />NOW THEREFORE, in consideration of these premises, and mutual covenants and <br />agreements hereinafter contained, the parties hereto agree as follows: <br />1. The Community Service Provider(s), as specified in the County Funding Plan on <br />the Provider ervi s umma format(s) (DOA -732) for the period ending June <br />30, 19 93 is (are) as follows: <br />Joint Oran e- Chatham Community Action <br />Social Services Department <br />2. The terms set forth in this Agreement for payment are <br />contingent upon the receipt of Home and Community Care Block Grant funding <br />by the Area Agency. <br />3• Grant Admini trati n.
