(801) 268.0553 <br />µi(I of tho Aetna hunt PUN W�, i <br />June 2, 1992 <br />Orange County <br />109 Court Street Annex <br />Hillsborough, North Carolina 27278 <br />Dear Gentlemen: <br />On behalf of Human Affairs International, Incorporated, I propose <br />to amend our agreement with you entered on February 6, 1990 (and <br />amended by a prior amendment through June 30, 1992) (the <br />"Agreement "), which established an employee assistance program for <br />Orange County employees and their dependents, as follows: <br />9' <br />1. The last sentence of Exhibit A, Paragraph Y of the <br />Agreement is to be deleted and the following is to be inserted in <br />its place: <br />HAI will provide these reports to Employer on an annual <br />basis. <br />2. The first sentence of Exhibit C, Paragraph 1 of the <br />Agreement is to be deleted in its entirety and the following is to <br />be inserted in its place: <br />For the period July 1, 1992 through and including June 30, <br />1993, Employer shall pay HAI at a rate equal to the number of <br />persons on Employer's payroll times one dollar and sixty -three <br />cents ($1.63) per month. <br />3. The last sentence of Paragraph 9 b. of Exhibit D of the <br />Agreement is to be deleted in its entirety and the following is to <br />be inserted in its place: <br />Employer shall allow HAI to continue to provide such <br />services at the rate of one hundred dollars and no cents <br />($100.00) for each hour or session of counseling (whichever is <br />less), or to make other clinically acceptable arrangements for <br />continued services. <br />
