15 <br />information including how to contact the Duke Poison <br />Control Center is distributed to each family. <br />Educational sessions are also provided to day care <br />workers at participating day care centers. Several day <br />care centers and the Kiwanis Club are collaborating <br />partners in this project. Any plans for continuation of <br />this project are incomplete at this time. <br />The Baby Love Maternal Outreach Worker program is <br />currently getting underway. The health department <br />applied for and was recently awarded funding for this <br />program which provides for community -based outreach <br />workers who will work in collaboration with maternity <br />care coordinators - from -both -the -- health- department- -- and -- - - - <br />Orange- Chatham Comprehensive Health Services (OCCHS), a <br />local community health center, and child service - <br />coordinators to provide intensive support to pregnant <br />women and their families. The goal of this program is <br />to improve pregnancy outcome and reduce infant <br />mortality. The program has been funded for three years <br />with funding for years four and five contingent on an <br />evaluation of the first three years. One worker will be <br />based at health department offices in Hillsborough to <br />serve northern Orange County and the second worker will <br />be based at OCCHS in Carrboro to serve southern Orange <br />County. Workers will also work in conjunction with the <br />health department's maternity care coordinator stationed <br />at UNC Hospitals' prenatal clinics. This project is <br />truly a unique collaborative effort involving a local <br />health department, a community health center, and a <br />tertiary care center to provide community -based outreach <br />services. <br />2) Allocation of Resources: <br />Eileen Kugler, MSN, MPH, Director of Personal Health <br />Services /Public Health Nursing Director will serve as <br />project director providing guidance in project planning, <br />implementation and evaluation, technical assistance and <br />monitoring, and reporting. Time allocation = 2.5% <br />The administrative office of the health department will <br />process financial transactions. <br />
