The agencies identified to <br />date to provide these services are as follows: Home Health Agency of <br />Chapel Hill, The Right Staff, Health Force, Medical Personnel Pool, <br />Triangle Home Health Care, Charles House, and Sharon's Way. <br />BACKGROUND: <br />In the recently approved Home and Community Care Block Grant Funding <br />Plan $23,043 in federal and state funds were allocated for in -home <br />respite chore services and $3,000 for institutional respite through <br />the Department on Aging. The local match of $2,893 has already been <br />budgeted. <br />The departments of Aging and Social Services have identified a need <br />for short -term relief for caregivers of clients who have little or no <br />funds to pay for this type of assistance. These are family situations <br />where the older member cannot be left alone due to a physical or mental <br />condition. Additionally, a need has been identified for the performance <br />of critical tasks in order to maintain self sufficiency within the home. <br />In order to spend all of the funds available and maintain a quality <br />provider pool, several providers will be utilized. The programs will be <br />administered by the Department on Aging but coordinated by the inter- <br />departmental Case Management Team, comprised of Aging and Social Services. <br />RECOMMENDATION(S): <br />Manager recommends approval and Chair to sign provider agreements. <br />
