The names and addresses of the property owner(s) and /or applicant(s), and the names and addresses'-arc all persons owning property within five hundred (500) feet of the property in question. Application fee as set by the Orange County Board of Commissioners. Traffic impact study as required by Article 13 of the Zoning Ordinance. Additional information regarding the proposed Special Use as required by Article 8 of the Zoning Ordinance. Narrative (or letters from appropriate agencies) indicating: 1. Method and adequacy of provision of sewage disposal facilities, solid waste disposal, and water service. Where public sewer is not available, a letter from the Orange County Health Department certifying the suitability of the existing and /or proposed wastewater treatment system for the property. 2. Method, and adequacy of police, fire, and rescue squad protection. 3. Method and adequacy of vehicular access to the site and traffic conditions around the site.
