of building a community organization and identifying community <br />r <br />leaders to participate in the project. y g y 4 <br />• The "middle" county in terms of community readiness is Orange County, <br />where there is a community group, but that group needed a focus <br />around which to build. <br />• The most well developed in terms of community readiness is Wake <br />County, where there is a strong, well- developed community -based <br />organization that is more than ready to take new steps in serving their <br />community. <br />• Each County's focus in proposal <br />-Lee Count has chosen to work with a local Housing Authority and <br />build the capabilities of two community organizations in two housing <br />projects in one of the cities in the county. This will be the first time a <br />health department has actively worked with a housing project <br />population in a broader context than simply health department services. <br />Residents have identified a variety of issues, including violence and <br />drug -abuse and the health department has identified teenage <br />pregnancy and maternal and child health as . priorities. <br />- Chatham ount will have as its lead agency in the initial year, the <br />community -based umbrella organization, that will be working to <br />strengthen the emerging community organization in a community on <br />the outskirts of a middle -sized town. During the four year plan, the <br />leadership of the project will transfer to the health department as its <br />builds its competence in dealing with the community as well. <br />Environmental issues as well as adolescent, maternal and child health, <br />and sexually transmitted diseases will be a focus <br />-Orange County will be using the Community Health Advocacy <br />Project (CHAP) model to organize and gain input from a rural <br />community in the county. Part of the plan is to hold annually, a needs <br />assessment and planning. forum where agencies and community <br />members in partnership work to identify needs and formulate action <br />plans to address the top priorities identified. <br />-The Wake Count y focus is on the development of a resource center <br />within the identified minority community in the city of Raleigh. The <br />purpose of the center is provide a physical focal point and support <br />services for efforts in the areas of economic development, leadership <br />development of community members, health promotion efforts, and <br />organizational support. Activities will include things such as, seminars <br />in organizing and managing small businesses, community health <br />education and screening efforts through lay health advisors, support <br />services for grant preparation, meeting space for groups planning <br />community programs, marketing assistance etc. The Wake proposal has <br />a half -time assistant health director and a half -time faculty from the <br />SPH devoted to the project, the first-time such a shared arrangement <br />has been tried. <br />
