2 <br />If approved, the Kellogg Grant would provide funding through the <br />school of Public Health for the Health Department, the Cooperative <br />Extension service, the Joint orange Chatham community Action <br />Agency, and occHS to establish positions to work in several <br />community -based projects designed to improve the health indicators <br />for minorities and other special populations at risk. The Health <br />Department and Cooperative Extension service would share a full - <br />time educator position to supervise a core Of community health <br />advocates, manage a steering and advisory committee, and plan an <br />annual community health needs assessment and intervention planning <br />forum in several communities in need. <br />Kellogg would provide funding for a four -year period during which <br />time the consortium will seek continuation funding from a variety <br />of sources. No commitment of county funds is required for the <br />application. The attached summary provides additional background <br />information on the development and current status of the <br />coalition. <br />The final application is due to be submitted by the school of <br />Public Health to Kellogg by April 30, 1992. A site visit by <br />Kellogg,will occur on May 14 and 15, 1992. A decision will be <br />reached in July for funding to initiate August 1, 1992. <br />RECOMMENDATION($); The Manager recommends that the Director of the Health <br />Department be authorized to seek funding for the CHAP <br />Program contingent on the following: <br />1. That the Health Department will evaluate the <br />effectiveness of this program by the end of the grant <br />period. <br />2. Determine the need for continuation of the CHAP <br />Program. <br />3• If CHAP is continued after the grant period, find other <br />. non -tax sources of revenue. <br />
