SECTION C--COMMUNITY INVOLVEMENT <br />1. (New Applications Only�—Attach three letters of support referencing knowledge of and support for proposed <br />project. <br />2. (New Applications Only- Describe how potential volunteers, or persons to be served, or their representa- <br />tives, were involved in the development of this application. <br />3. a. (New Applications Only) -- Specify your plan for establishing the project's Advisory Council. Member- <br />ship should reflect the population characteristics of the project service area and include volunteers, persons <br />served or their surrogates, representatives from ethnic and minority communities, the handicapped, and <br />community leaders. <br />b. (Renewal Applications Only) -- Attach a list of current members of the Advisory Council showing their <br />organizational or community affiliation. (See pg _ 15 ) <br />4. a. (New Applications Only)— Describe sponsor's plan for regular appraisal by the Advisory Council of <br />project management and the progress in meeting the project's goals and objectives. <br />b.
