GRANT PRE - APPLICATION Page 5 <br />MONITORING/EVALUATION CRITERIA <br />Begin below, add continuation page(s) if necessary. <br />The 3 projects will be individually- monitored and evaluated by measuring monthly <br />progress against the outlined measurable objectives and activities, and monitoring <br />progress of the proposed implementation timeline. Documentation will include <br />reviewing the following: <br />a. Written reports on families served and services provided by Clinic <br />volunteers at the Child Medical Evaluation Clinic <br />b, Evaluation forms from the two Clinic volunteer trainings <br />c, Evaluation forms completed by each support group participant <br />d. Evaluation forms completed by each participant in the trainings for mental <br />health clinicians <br />e.
