We are requesting funding for two <br />(2) MOW's: one to carry a caseload in the southern part of <br />Orange County and the second to carry a caseload in the <br />northern part - each very separate and distinct communities. <br />Our plan for the program provides a unique opportunity for <br />interagency collaboration among a local health department, a <br />community health center, and a tertiary care center. <br />We are very excited about the potential opportunity to offer <br />this program to Orange County women who qualify and look <br />forward to a favorable evaluation of our application. <br />SOUTHERN ORANGE OFFICE: Carr Mill Mall.
