r' <br />BABY LOVE MATERNAL AND INFANT OUTREACH PROJECT <br />STATEMENT OF ASSURANCE <br />(Name of Agency) <br />13 <br />agrees to meet the <br />following as a condition -for receiving Baby Love Maternal and Infarct Outreach Project <br />funds <br />from the Division of Maternal and Child Health: <br />(1) Beginning on January 1, 1992 the local project agency will commence with project <br />implementation. <br />(2) By March 31, 1992 the Baby Love MOW(s) will be hired. <br />(3) During April 1992 the Baby Love MOW (s) and .local supervisor will participate in <br />the basic training program provided by the Division. <br />(4) By June 30, 1992 each Baby Love Maternal Outreach Worker will have developed a <br />caseload of 20 -30 at -risk pregnant women (who have had MOW services initiated <br />prior to their 28th week of pregnancy) . <br />(5) By July 15, 1993 the first semiannual report will be submitted. <br />(6) By December 31, 1992 each Baby Love Maternal Outreach Worker will have developed <br />a case load Of 45 -50 families (consisting of pregnant women and maternal /infant <br />Pi's) - <br />(7) By January 15, 1993 the second semiannual report will be submitted. <br />(Signature of Agency Director) <br />(Signature of Fiscal Agent) <br />11/25/91 <br />(Date) <br />(Date) <br />
