10 <br />Review of current MCC cases in all three health care agencies <br />identified at least one of the above problems in over 50% of MCC <br />cases. in many cases more than one problem was identified. women <br />faced with these identified problems are the population that re- <br />quires more intensive services requiring additional MCC /CSC con- <br />tacts which.include'MCCs /CSCs providing services that an mow could <br />handle. In additional most of the needed services are not avail- <br />able through existing programs. <br />Not all women with one of the above problems will require more <br />intensive services as they will respond to interventions offered <br />through case management or are already linked to needed resources <br />and services. However, of the 50% identified, through review of <br />MCC cases, roughly half of those identified require intensive <br />services, which is consistent with the experience of MCCs state <br />wide. <br />In order to accurately identify and refer women in need of Mow <br />services; a guideline for MOW referrals will be established. MCCs <br />w1 ?. identify problems during the initial psychosocial assessment <br />and subsequent contacts with MCCs making MOW referrals in a timely <br />manner after identification of a problem, but prior to 28 weeks <br />gestation. The Mow referral must include at least one of the tar- <br />geted problems and must specifically indicate that intensive ser- <br />vices /follow up are needed. The established guidelines will serve <br />more as a method to target and prioritize the most difficult cases <br />but will allow some discretion based on specific situations. <br />service plans developed by MCCs /CSCs and Mows will involve working <br />closely with community agencies. service plans will integrate <br />accessing specific resources and services provided in the <br />community. In addition the use of volunteers will be incorporated <br />as needed (such as having a recovering substance abuser meet with a <br />pregnant substance abuser, utilizing other natural helpers, and <br />utilizing peer counseling services available in the community). <br />6. DESCRIBE YOUR PLAN FOR INVOLVING THE COMMUNITY AND OTHER PROVIDER <br />AGENCIES IN THIS PROJECT. <br />The orange county Health Department (OCHD) has previous experience <br />organizing advisory councils for our Family Planning Program and <br />our "In the Know" Minority Aids Education Project which is a <br />program using the lay advisor (Natural Helper) approach to reach <br />minority teens. The program utilizes teen and adult Natural <br />Helpers. As the coordinative agency for the Child service <br />Coordination Program, the department is also an active participant <br />in the local orange county Preschool Interagency Committee as well <br />as the interagency consortium for P.L. 99 -457. we are also <br />involved in several other interagency efforts. As a result there <br />is already a strong interagency network in the County. <br />Community involvement for this project would be obtained throuch <br />the establishment of a Maternal and Infant outreach Project <br />Advisory Group. The advisory group would be modeled after the <br />above mentioned councils and draw from our established networks in <br />6 <br />
