8 <br />All MCC referrals for an Mow will go to one of the project <br />supervisors for assignment. cases will be assigned by the <br />project supervisor to the Mows based on the caseload guide - <br />lines established by the project. once the Mow reaches the <br />maximum targeted caseload for a.1 given year, no new referrals <br />will be assigned unless an MOW client withdraws from the <br />program. Though some clients may not be able to take advan- <br />tage of more intensive services because of the limits on case- <br />load size, it is important to restrict the caseload size of <br />Mows such that project goals can be accomplished. The un <br />served population could be used as a control group when <br />evaluating the MOW project. <br />The mow will be an integral part of the maternity.care team. <br />After a referral is assigned to an Mow, the involved MCC and <br />MOW will jointly develop an MOW care plan, clearly specify- <br />ing roles /responsibilities of the MCC and the MOW. The MCC <br />and the MOW will review and amend care plans on a regular <br />(specified) basis. The Mow will offer continuity between <br />MCC and CSC. After delivery the MCC, the MOW and the CSC <br />will have a case conference to plan for the transition from <br />MCC to CSC. The Mow will accompany the Csc on the first CSC <br />home visit. The CSC and the Mow will jointly develop a post <br />delivery Mow care plan, clearly specifying roles/responsibi- <br />lities of the Csc and the MOW. The Csc and the Mow will <br />review and .amend care plans on a regular (specified) basis. <br />B. Mows will be assigned based on geographic location of the <br />individual being referred. Mows will be assigned geographi- <br />cally in order for them to work effectively within the <br />designated communities. One Mow will serve the northern part <br />of orange county and will be accountable to the project <br />supervisor at OCHD. The other Mow will serve the southern <br />part of orange county and will be accountable to the project <br />supervisor at OCCHS. Caseloads will be monitored between the <br />two supervisors to insure that the caseloads remain equiva- <br />lent. There will be joint staff meetings and staff develop- <br />ment for all Orange County MCCs and Mows on a monthly basis. <br />After delivery and once Csc is involved the project supervisor <br />at OCHD will assume supersory responsibilities of the Mow <br />since the CSC Program operates out of the OCAD. <br />orange county has a unique make -up of distinct and separate <br />urban and rural areas. The southern part of the county <br />comprised primarily of chapel Hill and carrboro is urban, <br />while the northern part is a rural farming area. The two <br />communities are very different in terms of needs of resources, <br />and the way each view itself in relation to the other. <br />it is for this reason that we elan to assign one MOW to the <br />southern area and one to the northern and plan to recruit <br />workers who will be a good match for each community. <br />4 <br />
