In the event there is a desire for the facilities to chance ownership, or there is a name change of the Pe.:nittee, a formal pe:-nn,it reauest must be submittea to the Division of Environmental Management accompanied by an application fee, documentation from the parzies involved, and other suppor-Ling materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. It. Failure to abide by the conditions and limitations contained in this pernit may subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 1 -43- 215.6. 5.
