PRIMARY PURPOSE: <br />ATTACHMENT 3 <br />Division: Personal Health <br />Proposed Grade /Step: 69 /Step hiring range <br />Proposed Hiring Date: <br />Number of Hours Per Week: 40 <br />Temporary: <br />10 <br />This position is requested to utilize permanent additions of funds to the state <br />Consolidated Contract in the Family Planning and child Service coordination <br />programs. The primary purpose of this position is twofold: 1) to provide <br />clinical nursing services in the new STD clinics in order to add family planning <br />clients thus expanding our capability to see family planning clients; and 2) to <br />provide professional nursing services to a caseload of approximately 25 children <br />and infants enrolled in the Child service Coordination program. <br />PRIMARY DUTIES AND MINIMUM - EDUCATION AND EXPERIENCE: <br />2. DUTIES: <br />This position will be .responsible for providing professional nursing services in <br />family planning and child service coordination in both home and clinic settings <br />to include: <br />- Developing nursing care plans which include assessment, planning, <br />implementation and evaluation. <br />- Performing client interviews, vital signs, developmental tests, screening <br />and lab tests. <br />- Administering medications and immunizations, dispensing medications as <br />specified by pharmacy laws, obtaining informed consents and providing <br />counseling about medications and immunizations. <br />- Providing teaching and counseling on a variety of topics in family <br />planning, STD, child development, childhood diseases and conditions, <br />parenting, health maintenance and other women /children's issues. <br />- Providing referral and follow -up based upon client needs. <br />- Providing case management to coordinate all needed services from a <br />variety of providers for a caseload of infants identified through child <br />Service coordination <br />- Maintaining client records and reports according to program requirements <br />3.
