While preparing the <br />copy -ready document, I discovered that the introduction page had <br />been inadvertently omitted from the copy I sent to each of you at <br />the end of August. Please insert the page attached to this memo in <br />the front, of your copy before the Table of Contents. Since I <br />received no comments during September regarding the form or content <br />of the guide, no revisions were made. <br />
