2 <br />CHANGE ORDER JUSTIFICATION <br />911 EXPANSION PROJECT <br />DATED: SEPTEMBER 25, 1991 <br />CHANGE ORDER REQUESTED AMOUNT OF CHANGE ORDER <br />JUSTIFICATION FOR CHANGE ORDER <br />1. Tree removal due to $400 <br />Environmental Health recommended a larger repair field <br />relocation of front <br />for the on -site low pressure septic system. Nail over existing roof on $300 <br />The roofing contractor installed the wrong color shingle on the new <br />old section of 911 Center. <br />section of the building. Since there is less roof area on the old <br />section, the County allowed the contractor to install shingles on <br />the old area which match the new addition, rather than requiring him to <br />remove and replace the new shingles on the new section. The net effect <br />is that the County is able to install a new roof on the old section for <br />$300. <br />5. Concrete stairs $1683 <br />These stairs will connect the upper sidewalk and the lower sidewalk. <br />The original plans did not call for the sidewalks to be connected, <br />however EMS personnel indicate that access to the EOC as well as for <br />FNS personnel will be greatly enhanced with the addition of these <br />steps. If the steps are not installed, you must access the lower level <br />by walking around the driveway and down the hill. <br />6. Level basement floor $1427 <br />Floor was sloped for drainage in the "garage" area of the exisiting <br />building. The slope was not identified until items being stored against <br />the wall were moved during the construction process. The slope begins <br />approximately 4" into the room and slopes for a total of about 6 ". If <br />the floor is not leveled there will be a 2" drop in the middle of the <br />room where the old floor level and the floor level of the new addition <br />meet. <br />7. Fill dirt over boulder in <br />east parking lot. $ 424 A boulder, approximatley the size of a small automobile was discovered <br />in the east parking lot during excavation. It was estimated that the <br />cost of removing the boulder would exceed the cost of raising the grade <br />of the sidewalk and parking lot. <br />
