4 <br />PA <br />STATE OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />AGREEMENT BETWEEN <br />WAKE COUNTY OPPORTUNITIES, INC. <br />AND <br />THE ORANGE COUNTY HEALTH DEPARTMENT <br />THIS AGREEMENT is made and entered into this the seventeenth day of <br />September, 1991, by and between Wake County Opportunities, Inc. of Raleigh, <br />North Carolina, for its Headstart Program referred to as "Headstart "; and <br />the Orange County Health Department of Hillsborough, North Carolina, for its <br />Dental Clinic hereinafter referred to as "Dental Clinic." <br />WITNESSETH: <br />WHEREAS, Headstart desires the services of the Dental Clinic to provide <br />dental services to the children who are enrolled at the Cedar Grove Center; <br />WHEREAS, the Dental Clinic wishes to provide dental services for <br />Headstart; <br />NOW, THEREFORE, in consideration of the premises and the following <br />mutual promises, covenants, and conditions, Headstart and the Dental Clinic <br />agree as follows: <br />1. The Dental Clinic will provide dental examination, assessment, <br />treatment, and education services for approximately eighty (80) children, <br />3 -5 years old, enrolled at the Cedar Grove Center. <br />2. The Dental Clinic will assure that its employees and contract <br />officers are covered with professional liability insurance while rendering <br />services to the Headstart children. <br />
